If it is the same as the Elite Snake then you just kill the little leafs, there is a small chance the elite will spawn when it needs to replace a leaf.

Go to the forest west of the farm, to the spot with the leafs and the injured person. Should be the first north path in the forest zone. You don't always need to kill the leafs for an elite to spawn. There are so many, that elites are abundant.
