How much am I really missing if I don't download the DLC
I have played around 20 hours before realizing that I needed to start a new game with the dlc for the extra content. I read that without it half of the game is missing but that seems too extreme, is this really true?

I'm not really far into the game but the way I see it, not much if anything at all.

One Dlc gives you music from SMT 1, 2, 4 and 4a, but only in the overworld so it's useless in dungeons.

Maniax gives you Dante. If you don't care about DMC then you can stick with Raidou.

Then there is the DLC that gives you an "extra" dungeon with the only purpose to give you extra money and EXP.

I don't really get what people mean by "without it half of the game is missing."

Isn't Maniax give our extra character (dante, raidou) and a way to true ending ( Kalpa dungeon ) ?

The fiends, Metatron, Belzebub, and True Demon Ending. It's not that important, if you download it later, you can NG+ with the DLC

Maniax add bits and piece into the game but 50% is kinda over the top. its around 10%.
what you get is more stories, the Amala Dungeon and the True Demon Ending and also the Fiends. Amala is a pretty long dungeon which unlocked with every defeat fiends its a great place to lvl up mid game if i remember right.

Not too much. The dlc is just music and grinding maps, just use merciful if you want quick grinding and switch back. Dante is pretty much the same as the Raidou dlc just with Dante instead.

you're kidding right? You have to pay the extra $20 to get a true ending?

Calling it "the true ending" is misleading. Every ending is as valid as any other. It's called "True Demon Ending", but only to contrast against the "Demon Ending", which have different conditions for triggering, anyway.
The original Japanese release didn't have the True Demon ending until the "Maniax" edition rerelease on PS2, either.

No because the Chronicle DLC with Raidou is free, a fact which people seem to keep getting wrong. Maniax is literally just paying for a Dante skin.

That's technically only half-true, as IV A is a altering the course of the IV Neutral ending with the introduction of Nanashi's actions.

Nope in smt games, the neutral endings are always the true Canon endings. It's always been that case. You new to smt or something?
Tldr true Demon Ending is the true ending

That's a misinformed take; "Always" is SMT1->2 and 4->4A, but in Nocturne's case, the whole LNC-dichotomy is thrown out the window. Hell, the "True Demon" ending didn't even EXIST in the first Japan-exlcusive release of Nocturne.

true, it was added later, but it's still considered the canon ending. Also, You forget the other smt games? there are more than just the numbered ones. For example, soul hackers, strange journey, devil survivor 1 and 2, digital devil saga 1 and 2. Probably forgetting some others. The neutral ending is the canon ending in all of those as well. It's a mainstay, it's a fact. Every smt fan knows this. You obviously aren't a true fan, you didn't even know about the other games lol. Please don't try to speak with any authority on a topic you obviously know nothing about. If anyone is misinformed its you.
