Everything I've read says that you need to have the Forgotten Lands unlocked to start finding the Dolls. Do you JUST have to have unlocked the Forgotten Lands, or do you need to do something specific there first?

i think you just need the Forgotten Lands unlocked. I have Jack currently but haven't restored the pillar yet.

The Dolls started spawning for me after I completed the first quest for Fairy Godmother. So, that's the answer. You need to unlock the Forgotten Lands and complete Fairy Godmother's first quest.
Edit: I found all 4 and the tree within 10 minutes of completing this quest, after a day and a half of sweeping the valley multiple times with no luck.
