How do bathrooms work?
It's the whole "ate without a table" thing again, I have 40 bathrooms, but everyone keeps soiling themself

Please submit an in-game bug report, I'll tell you the reason. Usual suspects - issues with oxygen, temperature or no path. Use the temperature / oxygen overlays, also select a colonist that fails to use the toilet, hold CTRL and hover the toilet area to see if pathfinding can find a path.
Every bedroom contains a sleeping pod, a toilet, and a shower. The auto-clean on all are set to 20%. At some point I theorized that maybe they needed to use a toilet but couldn't make it to a bedroom in time, so I sprinkled some more toilets around the ship. In total, there are 102 toilets. None has more than 1 unit of bio-waste.

Two colonists soiled themselves in the last day. Daniels soiled himself two hours ago and is now sleeping in the bed next to his toilet. His toilet has 1 unit of bio waste, so it is definitely usable (because it has been used).

Here is another example.

Paris is a mess. She soiled herself two hours ago. In the last day and a half, she ate raw food without a table and had nowhere to sleep.

She has a bed assigned to her, and a clear, safe path to it (and an empty toilet in the same room).
She also a clear, safe path to a fridge full of soy burgers, survival meals and bread.
She also has a clear, safe path to a dining table.

As an experiment, try taking the toilets out of the bedrooms.

OK, I took a look at your save, the problem will be in the scale of all things. I rarely see ships that big. I'll run some tests and use your save file as a test bed to improve AI behavior in huge ships.

I took a closer look, and it seems like your colonists sometimes soil themselves because they couldn't make it to the toilet in time. You ship is massive and walking through it takes a while.

My suggestion would be to have toilets, showers and food facilities distributed in a way that they can be quickly reached from anywhere. Also in this situation a central teleporter hub and spokes in each zone could work, unless you have pathfinding optimizations for teleporters disabled (which could improve performance in this case).
