Buddy, me and my mate had the same issues when we are playing together. I don't have an exact answer to you but through experimentation we did, I can advice to you the following that will help you reduce the "unauthorised" penalties. Also consider the fact that this game is in early access so it can affect your experience too, bugs wise...
You should learn the currently number one rule when it comes to suspect surrendering. If they surrender with the weapon in their hands (meaning they lift their hands up after you shout for compliance with the weapon in the hand), It considered a fake surrender and you can engage the suspect without gaining any penalty. If the suspect drops the weapon AND lift their arms up (empty-handed) then it is a real surrender and you should definitely not shoot the suspect. Do be mindful of surrounding suspects that can either shoot you or the surrendered suspect.
One big factor in receiving this penalty is engaging suspects from a high ground (meaning you are a level or two above them), We have notice that even though you shout for compliance and the suspects do a fake surrender. If you shoot first, there is a high chance you will incur a penalty. Your best chance when it comes to engaging suspects from high grounds is to let them shoot first (you usually have enough time to move backwards to safety between the time you shout for compliance and the suspects stating to shoot at you) and then you engage them. Of course you can always attract the suspects' attention and ambush them at a staircase entrance.
Going back to the fake surrender, you can shoot the suspects' trigger hand (The hand they use to shoot the weapon with) with 2 bullets when they lift their hand up to make them drop the weapon, but be mindful of several stuff. The is a delay (I assume due to coding of the game or the animation) from when you shoot the suspect's hand and them actually dropping the weapon. What usually happens is that the suspect will shot a few bullets and then drop the weapon. You should always be mindful of your environment when engaging suspects like this as you can shoot their hand and then duck to cover, let them do the few bullet shooting thing and then they will drop the weapon and you can arrest them without penalty. Or shoot them, up to you.
If the suspect did not drop the weapon after you shoot them 2 bullets and you still want to arrest them. wait behind the cover for the suspect to reload, lean out and shot their hand again. it will force them to drop the weapon for sure. Don't shoot a third bullet into the suspect's hand during the fake surrender animation if you plan to arrest them as it will definitely kill the suspect.

I'm almost certain I got uuof almost every time I shot someone fake-surrendering, so I guess with your observations in mind I'll again do some testing myself sometime soon.
If I understand you correctly, you seem to suggest that terrorists can survive three bullets if two of them are fired while fake-surrendering and one after they've shot back. I'll have to try that one out, too, as it always seemed to me that them not surviving more than 3 bullets is the only constant in the game.

Additionally, I think that distance from the suspects has also the same effect as elevation. Meaning if you are far from them it's better to let them shoot first after you shout for compliance. We did notice that from far away if we engage them first while in fake surrender, we will get uuof almost 100% of the time (Paradise City is a good example of this).
I am not sure but I think that when you shoot during the fake surrender animation is also has a factor but I'm just speculating. If you shoot the hand after shouting but before the suspect lifts it, I think they will drop it. But if you let the hand go up with the weapon, it locks it in animation or sequence where the suspect will not drop the weapon and will continue to engage you for a few rounds before it registers and they drop the weapon.
