ME2: No one left behind bugged?
I followed a guide to unlocking the achievement, and i thought i had made a mistake by doing legions loyalty mission before the collector base assault and ended up getting half of my crew liquified, though in the same guide it states that it is inconsequential to the achievement. I have all three ship upgrade (ie shields armor and cannon) I picked legion to be the tech specialist, he was loyal and survived. I picked garrus to lead the first fireteam, jacob to lead the second, made mordin go back to the ship to take the survivors back, used loyal samara as biotic specialist and then left both garrus and grunt to hold the door, all of which were loyal, survived, and met the requirements for combat specialists so i am very confused here. Are the crewmates required for the achievement? Or is it just your recruited team?

Was everyone loyal? I know not all have to be loyal but it's best if they are because non-loyal squad members can die, even if you make the right choices. Non-loyal squadmates you take to the final fight will die.

You should be able to do Legion's loyalty mission after installing the IFF without issue. You can actually do a couple of missions after the IFF Installation without losing any of your crew, though they don't count towards the achievement.

If you did all that, it should have popped. If not, it could just be bugged and didn't register.

Yeah, i even did a double take and looked at all of the squad member locations after the fact and they are all very much alive, all members were loyal as i completed all of the loyalty missions and i made sure to put the right people in the right places. Additionally there weren't any deaths in cutscenes. The only mission i did after IFF was Legions loyalty mission, and it killed half of the crew for some reason, but once again, guides seem to point out the crew member deaths don't count towards the achievement.

Then it sounds like you bugged out. Perhaps you briefly lost connection to the internet or a hiccup in the servers kept the achievement from registering. Do you have a save from just before the final boss fight to try again?

The funny thing is after the mission i got an achievement but it wasn't Leave no one behind, so it couldn't have been service related, so i'll try again and post an update later, haha.

Edit: I just realized how stupid i am. The achievement unlocked on steam, but the reason it didn't show in the in game menu is because it did not in fact unlock for EA, my name speaks for itself lmao.

Yep, so sounds like there was a hiccup in EA's server when that achievement popped. I assume it also unlocked in game when you check your achievements from your cabin.

Use the trilogy save editor, and open the last save. It should tell who survived and who did not survive.
