Are mods for ME3 worth it?
Should I install them or should I just play it vanilla?

First playthrough should be Vanilla.

After that, I recommend a Vanilla+ playthrough. It's modded but sticks to the lore. There is a great and easy to use modding guide here for an excellent Vanilla+ Experience

Take Earth Back mod for the final sequence having more assets that you acquired actually show up is a nice touch.

Expanded Galaxy mod, Project Diversity (OR diversification project), Take Earth Back, and the community patch are pretty big improvements over the vanilla without any immediate changes.

Extra note: If you're planning on only playing the game once, then I strongly advise the mods mentioned above to enhance the experience.

I have a few mods for 3, but less than the other two. There's a mod that lets you change your appearance and hair and whatnot from the armory, I really like that one for changing hair styles or having renegade scars when/if I want. 3 also has a mod that adds all of the armors from the 1st game, which to me just makes sense. There's an unofficial patch, and some consistency mods. Without mods, your Liara is going to look different in each game. Other characters too, like Mordin.

People who don't mod at least for consistency are missing out imo. Also in the first game, if you want to actually do a full complete playthrough do yourself a huge favor and install the Charted Worlds mod to have map markers for quest stuff.
