I beat otakamaru twice there. If you fail it helps that you start at the shrine again so you get full elixirs and can alter your loadout if necessary.
I stayed on his feet as much as possible and rolled/dodged around to his back as he cleaved etc. Timing my avoidance to his cleaves I could usually avoid 3 out of 4 and low stance block saves most of the one that sometimes lands. I can take his AoE atomic drop guarding even if it wipes me out (I have faster recovery) - but I can usually manage to hop back a bit (and behind or aside if possible) when he telegraphs that to avoid most or all of it while low guarding. It also helps to be in his face because you can easily be ahead of his shackles as he throws them out in front of him past his body so they will start past you above your head and fly outward away from you where they won't land.
I kept hitting him with soul core abilities and follow ups with ki pulsing. One of my soul core abilities is a distance closer which helps. Landing a counter helps and it's a lot easier to do if you are up close to him to start with. Burst counter usually refreshes one or more of my soul core attacks too (along with health and ki) so I start the cycle again. Going into yokai form (brute in my case) also recharges my soul core attacks so it's not always best to save that for last like you'd tend to do in nioh1. I did have to intermittently sprint up to him once I changed to yokai form though. He can be a boss that avoids up close melee, dropping back when not doing his thust/charge forward attack sequences and ranged attacks.
I think I only used a few rejuvenation (small health regain over time) onmyo scroll and 2 or 3 of the barrier ki regen ones, especially ki to get through the dark realm part. I think I loaded one of each at the start but I didn't stand there loading ornaments on a xmas tree with a ton of things. I have 9 elixirs (1 extra from the ninja tree). If it's your first attempt on him you also can have a bunch of elixirs and even a soul core laying around on the ground but after that from the shrine it's just otakamaru over and over until you beat him I think. I used a brute force attack style keeping pressure on like I said and I get health back on attacks so that worked together well with the assault strategy. I have ninja feathers and I'm using lightning on my weapon but I think I only hit him once with the feathers at some point. Mostly it was being aggressive with soul core attacks and follow up combos with plenty of flux1~2 ki pulsing and not letting him dance away.. staying right on top of him even if I had to burn some ki with intermittent sprinting because he can be elusive.
So now I finished both of those conveyor belt of opponents in a small arena levels and the cat spirit guide in the loading screens is saying you are so impressive that you'd probably get to the bottom of the underworld so I'm guessing that I'm near the end of the main game to where I just do underworld gear grinding. I put my pick up threshold higher already so I left a ton of garbage gear on that beach arena "room" in each conveyor belt of creatures level. The kodamas never gave me anything good in exchange really either even though you see them several times on those levels.
