The private bedroom and spacious bedroom are not stackable. You get whichever is highest (i.e. if you have a spacious bedroom you will not get the private one because spacious is basically a higher 'tier' of the same buff).
Without seeing screenshots of your rooms, I can't really comment on the why's and why nots beyond the above info. Though I can speculate that the 2 x 12 is too narrow for a "spacious" rating. There's even a debuff for "cramped quarters" when there's not good room to get around furniture and what not.
If I could get the 2x12 to work, I could just shove all the furniture against one wall and leave the other wall for pathing, but the game won't even recognize it as a private room despite it being the only bed in there and assigned to a survivor.
I noticed it when I added a bit more furniture in the kitchen/dining area of my last game. Suddenly they all started getting "cramped quarters" debuffs whenever they were in there with other people. The kitchen was 6 x 6, so not small at all, but with all the furniture, it was considered cramped.
They don't seem to mind my long dining room.

You may have any room design thus be the 2x wide or whatever else.
A bedroom is a room containing a bed. When the room is over 20sqm (or 20 tiles) then it gets the "Spacious Bedroom" buff.

I think the question is, if you had a 2x12 room in real life, would you think it spacious?
Hell NO! 2x12 room looks more like a private cell than a room
