EDIT - I should state this issue isn't just when they're moving from one building to another. It can happen when they're outside and trying to move into a building or vice versa. Nothing is in their way - they're the only one in the building or going to it and nothing has changed in the layout since the last time they successfully were able to get there. It seems pretty random to me, but maybe I'm missing something?
The issue isn’t consistent - Katina was asleep in there previously without issue and you can see Jack is asleep in the room so I don’t see that being the issue

I had just ordered Rita to play the handpan. There's clearly nothing at all blocking here. She was just in there and walked out and now seems to can't find her way back in. The only way I can get her to go there is to draft her, move her into the building, then undraft and reissue the command. The game has become unplayable because of this - can't play a game where 6 survivors are at random unable to exit/enter a building.
One thing I've noticed is that power grids are really weird, and really buggy in this game--including being able to remain powered after connecting poles are removed. Or when I have to disconnect the generators from everything else in order to get new add-on rooms powered. It would not surprise me if there was some kind of pathing interaction as well.
The issue appears to be the doors but the fix isn't permanent. If I move the doors over by just 1 cell, everything is fine for a few days. Then, the issues reappear and if I don't move the doors again (usually just shift them back to where they were previously) the issue spreads to more survivors and becomes more frequent. This seems like a pathfinding bug to me and I reported it inside the game's bug reporter. Thanks!
