What I did for the cap is just let them sit in the indoctrination portal. If a follower dies and you don't want to use bones to resurrect, then you can take a follower from the portal.
At least this way there is a pool and no known penalty other than a crapload of pending followers.
You can also get necklaces to double the life of your followers, which makes things slow down for death count.
Followers don't age if you haven't officially let them in yet right? You can just re-do dungeons over and over and over again until you have a ton of pending followers, then let them all in at once.
Unreasonable, yes, but perfectly fine to challenge yourself to get and maintain that many followers. Though, while there isn't really a cap on how many followers you can have, having too many followers could be a real struggle. I don't know how you would be able to maintain that many for long.
