trouble getting the 6 Feet Under ending

Every walkthrough/guide I've read just says you need to pick up the large key from the belongings drawer and the game will progress on its own. I keep seeing mentions of mannequins and a hatch appearing in the cold storage floor, but haven't seen any of it. The key is one of the first things I grab each shift.

It's a random spawn chance. It took me like 8 runs to get the closure ending to spawn.

There's another way to get this ending, but you have to piece the clues together from two separate events that don't necessarily occur in the same run. Ultimately, you're trying to figure out the combination to the lockbox in the cabinet on the floor to the right of the window in the embalming room. This box contains the key to the hatch outside.

1. There's an event where an apparition of a young woman appears in the bathroom and leaves a photo on the mirror. The writing on the back of the photo mentions a date. The year of this date is the first four numbers.

2. Sometimes, when you're about to inject the cavity fluid into the body, blood will drip from the ceiling onto the body, and you're transported to another location. On the far wall "EST. ____[number][number]" is scrawled on the wall in blood. The two numbers are the last two digits of the combination.

I'm not sure if they're 100% necessary to trigger the events, but I picked up both the large key and the tape recorder at the beginning of the run in which I got 6 Feet Under. (I usually ignore the tape recorder after the intro mission.)

I have yet to encounter the event with the mannequins in the basement myself, even after more than a dozen runs. Hope this helps!
