Report a translation problem
There is a bug with unknown cause that can jam the 'haunt system' though, which is responsible for the various events around the mortuary and also can impact the appearance of the 3rd or 4th sigil. Not extremely common, but does get reported.
Sometimes exiting and using continue will fix it, although you would have to re-embalm the 3rd body again most likely (the most recent auto-save).
Bugs involving vanishing sigils should have been resolved quite a few versions ago (at least, none have been reported since to my knowledge).
There's even a fail-safe built in that should they vanish, getting another letting strip and showing it / putting it away again, will attempt to make any sigils that should be visible, to appear again.
If you have more information about the sequence of events, recordings, or save files that can reproduce any such problems, they are needed to make any attempts to correct problems.
The fail safe might not be working because the sigil frame is still there but its blank in the center. If it happens again I'll report it. I didn't think much of it because I still had the name and remembered the sigil.
If it's there it should have one entry inside called SigilsReactivatedByPaper set to the value true.
If the file isn't there or the value is false, then even in later versions the bug may continue to occur (since it's a game engine bug that's being corrected by the game's code).

Maybe it was connected to the halloween decor since it only happened around then. either way its only happened once out of like 20 shifts or so. I dont know how programming works at all.
It's good that it doesn't seem very frequent at least. The earlier problem was happening way too often.
