Thanks for giving your take on it! To me, it sounded like the dad got attacked on the beach, rather than him losing his balance. Mainly because of the sense of confusion that comes before the 'oh ♥♥♥♥', and the extreme speed at which he falls (however, the latter could also be an amplification in Rebecca's memory to serve as a jumpscare effect for the player).
As for the headbashing scene in hell being nothing but a hallucination: could be!
It's highly likely to be essentially nothing but a hallucination since Rebecca deffinetely blames herself for her fathers death and it's something they mention quite alot how the demon will try to use "the darkest parts of you" against you.
I'm personally cautious to take alot of the visions at 100% face value since it's unclear how much might've been "embellished" for a lack of better words, by the demon, but over all I agree with SayYouLoveVi. Her dad likely lost his footing and smashed his head on a rock when comming back. Rebecca blames herself for his death because of her, at the time, drug problem which eventually escalated to causing the overdose she was suffering when her dad finally found her, leaving her helpless to essentially just watch as he died
