the first is definitely more likely and technically accessible for the actual software i would assume, but i would like either. think it would be fun.
In regards to enemies, maybe buff the ground enemies and make the fliers less numerous.
Maybe buff the bosses so they don't die that fast too.
As a new player, the experience is just very... "Oh, cool, I have 100 health." ... "Oh, a fly farted on me twice, now I have 50 health. Huh." With health point icons, you would immediately think "Okay, so I can take that many hits. Good to know." And as a new player more accustomed to FPS games where health actually lasts a while, you'd feel less irked at dying after four hits - the game told you how much you actually had, after all.
The 100 health number implies enemies deal variable damage - like Doom or something, where a little demon or gun zombie might deal a little damage, while larger enemies might hit you for more damage. It's not really appropriate for a game where almost everything deals the same amount.
Someones probably said it already but a new mode with more beats (that go to music possibly if we disregard how fast gun go bang/reload which the minigun kind of does overall so I don't see a huge problem in it) to the cross hair or alternating beats to the guns and abilities/powers having their own 'combo beat or/or movement to beats' you'd have to pull off all together to use it along side the more beats to tap to from gun alternation/game itself - I feel like that would the best game changer and add so much in a small way - still keep old classic beat style but I really can't play this for too long sometimes because of that simple cycle of beats even though it is so fun and already hard as balls sometimes.
I also don't even use Ultrawide support but if that hasn't been supported already - please make it happen. Pretty sure for the most part its simple enough to do, from what I've for people having to do it themselves when a Triple A title doesn't support it. Not comparing though just pointing out that happens and communities usually have to fix such a, usually small problem. Surely someone tell me if I'm wrong about it being simple as I'm not too sure, but modders and friends who have done it themselves would say otherwise to it being hard.
A setting to an optional gun to alternate to would be pog af... also ᵖˡᵉᵃˢᵉ ʷᵒʳᵏˢʰᵒᵖ ˢᵘᵖᵖᵒʳᵗ... ⁱᶠ ᵖᵒˢˢⁱᵇˡʸ, ᵖˡᵉᵃˢᵉˢ.

Online co-op likely can't be done because of latency. Your co-op partner would be shooting offbeat or be massively desynced.
Have a remaining ammo counter near the center of the the screen, allowing you to see exactly how many shots you have left before having to reload. Have it be a toggle in the option's menu for those that don't want/need it.
Have it be either above or below the metronome in some fashion.
As is, one needs to take their eyes off the center of the screen where the action is happening, to the very very bottom right.
Auto-reload when entering new room.
I feel this one wouldn't actually be as necessary if one could more easily see/notice/be reminded of not having reloaded your weapon.
I also think Barren as a floor modifier flat out shouldn't exist, it's basically just an instant restart if it appears near the start of a run.
