see if vanilla terraria has the same issue,
reinstall tmodloader,
update GPU drivers, update O.S, close all other processes other than TMOD before you launch it,
see if your GPU allows system wide vsync / freesync, if not, see if you can enable vsync / freesync via GPU drivers to a specific game,
then if that didn't work, then try to disable vsync in all ways possible, (gpu drivers, in-game, etc...)
run the .exe in compatibility mode, assuming you're on windows.
try to switch the cables that connect your monitor to your pc, see if it changes anything, if your monitor supports VGA / DVI or HDMI or displayport, switch between them and see if something changes.
