1. The "rescue hostages" missions may not spawn correctly rarely. It happened to me 3-4 times in a whole playthrough. (Hostages spawn without their handcuffs and when you kill enemies you can't rescue them and get the bonus)
This bug happened to me in areas that were near Copeland's camp and one time it happened when I fast traveled to a Nero checkpoint.
2.If the players burn down the final Nest in a infestation zone during an enemy grab sequence, the QTE will remain on screen.(Since the game brings the mission accomplished screen and that makes this minor bug)
3.I got a crash after a cutscene which corrupted my latest save and forced me to start the mission from the beginning:

4.When you clear out the horde in sawmill, the game doesn't give you the bonus for killing them until you finish a story mission.
5.One time the game always assumed that an enemy is near me which prevented me from saving the game.(And it forced me to replay a section)
This happened in the Rippers area after clearing out the Marauders camp at south.
Just wrote them here in case you wanted to fix more bugs.
I think you can also improve the game in some ways:
1.Give Deacon an option to slide down the ladders since climbing down the ladders is a little slow in this game.
2.Give us an extra option in the options menu to allows us to disable the "focus mode" since that is kinda OP in this game imo.
