If it's the one JadeGaiden has suggested, there's a narrow opening into another part of the mine. It's up a short tunnel on your right when running away. Better to find it before you free the hostage so you can find it when you run. By the way, if you just run as soon as you free the hostage, you can get away. If you squeeze through that opening, the freakers can't follow. You can stay and shoot them through the opening until it's blocked by bodies. Then follow the tunnel, bear left at a cross tunnel. You come out in the main tunnel, on a wooden scaffold above the freakers, and bomb them from there. They can't get to you there either. Either jump down and stroll out, or go back into the higher level tunnel and continue straight. It brings you out above the entrance you used to enter. You might encounter a few freakers on the way, but not the horde. You can also get back in the same way, if you want to refill your ammo, molotovs etc, and come back to finish them off.
If you're having game problems, can't complete the mission, which is just getting out alive. If restarting the game and doing it again, restarting your computer and doing it again, and while the game is off, verify game files. If these don't sort it, set all your graphics to low, including frame rate capped at 60 or 90. Play it through at that, then crank them back up. You should get through it fine.
I think the 'mission accomplished' trigger is starting your bike once you're out. But depending on where you leave it, it might be outside the mission zone. I've never had this problem, so I'm not too sure.
