Game will be perfectly smooth, sad that developers are so lazy to fix all of this.
Without mods I had persistant stutters too, no matter what settings and resolution was selected.
Bad part is - you can't play online with mods, but your gaming expirience will be on the next level, because play with 60fps lock and stutters is suffering.
Also good thing is use custom PureDark DLLS mod, which stopped to work after 1.10.1 game update, but worked fine on 1.10 and was increasing fps significantly. DLLS mod isn't free, but if you can google you can find leaked version for free. So I guess mod will receive update soon.
My PC is 12700k + RTX 3080 + 32gb DDR5 + 980 PRO NVME
I have the exact slowdown issue with Lords of the fallen but the difference is that that game wont throw me to offline mode.
It takes about a minute and my system soldiers through, during the slowdown i'm unable to get into task manager so i'm not able to see what hogs my resources.
Also noticeable is that it always happens only once after about 5 minutes in game, after that i can play trouble free for hours in Lords of the fallen.
Sadly because Elden ring goes to offline mode and can only go back online by restarting the game i'm unable to play online after it happens.
I can read my temps on my AIO and the cpu wont go up in temperature and the gpu temp drops to idle temp as its not being used during the slowdown.
Freshly build windows 11 system with almost no software besides gaming related.
Corsair ICUE, Afterburner and Sonic Suit Companion are running.
RTX 4090 with a 7800x3d
Did you try to revalidate your files/reinstall or maybe even clean-wipe-reinstall the game (except your savegames, of course)?
Maybe the game options are reset too.
It just seems a bit surreal that someone with such a behemoth of a computer has these issues out of nowhere, i run a rather low-end computer and the update changed nothing for me.
Can you tell us how to install the mods or give us a link with the instructions, i buy the game a few weeks and is really sad that i try everything for optimize the game without success i don't care of playing online it's doesn't affect me the multiplayer.
did a fresh install of Elden Ring, tried turning of windows 11 gaming settings and close corsair icue etc. Removed OC's from gpu and cpu, bios is up to date.
Did all the troubleshooting i can think off so far.
Like i said i had it my whole play through of Lords of the fallen but because it didn't throw me offline or anything i just took it as a bug in that game and didn't mind it as much. PC gaming has its quirks every now and then.
But now as of today Elden ring is unplayable for me online and i love the online aspect of from games.
Plus as i can think of these are the only two games i own that use Easy anti cheat so i think its triggering something in my system for sure.
I just played through Alan Wake 2 and i'm in the middle of Baldurs gate 3 without a single hitch.

I have the same mobo as you and this fixed it for me for Elden ring and Lords of the fallen.
A quick troubleshooting chat with Dark Breed showed that most were using an Asus board as overlap between the issue.
