On my second playthrough of the game it took me ages just to work out how to unlock the ability to level up. I couldn't figure out how to get Melina to appear, and thus by extension I couldn't get Ranni nor the bell either.

I don't even remember what I did that eventually made Melina appear. I believe it was getting all the way to Caelid at level 1. I presume there was something very simple in the early game that I was supposed to do and didn't do so she never triggered.

The game is just like that. It assumes you're going to do specific things, and if you don't do them, you're stuck without vital equipment or game functions.
sit at 3 bonfires in the open world in limgrave is the normal trigger

Yeah, that about adds up. I mean that isn't a thing I did that time.
I skipped the first grace with mr maidenless bloody finger, basically teabagged the grace in the chapel trying to get Melina to appear to no avail.... then just sorta gave up and meandered randomly east until I ended up in Caelid. I took the low route, didn't go into the first church of Marika or whatever, meandered my way up the hill trying to avoid everything trying to kill me, and ended up at that other Caelid village place with all the nasty blob things and near the crowd of loitering undead on the road.

I think it has to be "night time" in the world too in order for her to appear.

The first time I played Elden Ring blind, without looking at any guides, and I completely missed the spirit summon bell for my entire playthrough. I'm still not sure how you are supposed to know to go back there at that specific moment especially when it's night. Seems like too important an item to be that esoterically hidden.

You are the type of player that needs "yellow paint"

it doesn't, it just gets set to night when she shows up.

It automatically gets night when you go back there.
its also easy to go back since when you play without guides you know there is a smithing table to upgrade weapons.

why would you not go back there ?
you find upgrade material and you know there is a smithing table.

At that point in the game you dont know that later once you did sit at the Margit Bonfire there will be an invitation to a hub
So naturally 99% of players will go back there after finding upgrade materials
to upgrade the weapon for the first time.
and this automatically triggers night time and Ranni appearance

IIRC it automatically switches to night time when you hit the # of sites of grace, and Melina offers the accord, but if you just keep playing and don't go back to the church in time, it will switch out of night and "Renna" won't appear until it is night time again as a condition of her presence.

I have also had the issue of her not spawning for awhile, I have gotten the bell on atleast 13 characters but for 1 of them she just would not spawn and the store would not sell it, after beating margit she finally appeared, that run was weird because Melina would not spawn at that grace near the gate with the spear dude either but she spawned at the one down the road a bit. After she also took much longer to bring me to the table, I killed margit and the sleeping dragon at the fort grace and she would not bring me there until I sat at the grace where you get latenna, I think that character was bugged or something.

I accepted the accord at gatefront in my first playthrough, there are no upgrade materials there. I then proceeded to go away from the troll, and got rocked by the night cavalry, which reset the game to daytime. At that point, Renna will not appear until you cycle back to night.

it doesn't turn to night when mel shows up, "renna"s arrivial does, and will stay that way as long as she remains there, the moment she leaves, returns to normal.

simply not a requirement for her, just a trait of the event.

all she needs to show up is just having the whistle, and going to the church via warping, if you manually go there, you're gonna need to die or warp to get her to show up.

a lot of people make the journey back and forget that
