In looking at your time in game, it would seem to me you are kind of rushing it. Things are only going to get harder, much harder in the further areas. My advice would be to spend some time and complete all of the Limgrave and Weeping Peninsula content for Levels and Gear and Skill before launching into more difficult content.

Godrick the Grafted is about as easy as it gets in terms of real Boss fights.

When you can defeat the Crucible Knight in the Stormhill Evergaol in Limgrave, then you will be close to ready for the Lakes region.
