Patches missing
I played through the entire game, never met patches once. Somehow none of the triggers made him appear. None of the known locations had him or his drops. After defeating the Elden Beast, I finally found the Murkwater cave. He wasn't there either and the return to entrance waypoint was already triggered yet Patches was never killed. He is the only NPC in the game I didn't get the chance to meet. Has this happened to anyone else?

If you go to Murkwater Cave after completing Volcano Manor, he will be gone.

Yes it happened to me. I did things out of order. You really have to follow his quest pretty closely.
