Go south, Cross the bridge (of sacrifice). Clear the (morne) castle.
Once you cleared past the bridge, you'll never come back here ever again.
Go east until you reach a dying looking area, then southeast and clear the next castle.
Then you should definitely be strong enough to clear the main castle in Limgrave. If not, avoid it via a semi-hidden cliff pass on the east of the castle and do some of the lake area.
Also, level Vigor. Learn the enemy movesets and your attack and dodge timings.

No, you can get on your horse and rider past 90% of enemies without having to fight them. Then you can do one of the techniques to farm runes and level up.
So no, you don't get horribly wounded. Of course, you can always sneak attack it like others mentioned and I believe there is a skill that can even pull them off to stab but I'm not sure which skill it was so you would have to look it up (I think it is gravity related tho).
Yes, it has to be timed otherwise you get hit and fail to parry.
