Should I be taking notes?
Usually when I play an open world game it has stuff like quest tracking and the quest givers will remind me of any details I may have forgotten. It seems like elden ring is not going to do it for me however, so maybe I should do it?

If you want an immersive experience, yes. If you don't care, just use wikis.

eh, there isn't really a lot you need to remember to actually do everything
every once in a while a character might ask you to do something, or hint they want something, but its not really hard to remember the few things you need.

but if you are the forgetful type, it might be for the best.

I didn't use much of any guides and it's somewhat easy to accidentally get locked out of some quests by accidental progression. On my current NG5 I still have yet to Hand over some item to this guy during rannis questline to get the magic scorpion talisman which I have to do. Also I never got ansbach gear from the DLC. I've ventured too close to shadow Gate which locks out a fair but of content in thr dlc.

With that, I'd still not advise too many wiki guides outside of specific things you may want to accomplish or items you want to get. I honestly enjoy getting new items and leveling up items well into my 6th run of the same character and it's kept the experience relatively engaging.

You can place different markers on your map. You have key and info items in your inventory that remind you of some things. If that isn't enough, then yes, make notes.

Typically, most NPCs get clustered into their own side quests. Many side quests pertain to only one NPC for the most part. Only two side quests have overlapping portions.

Just keep an eye out for NPCs and read any letters/missives they give you and soon enough you'll figure out what to do and where to go. Also, don't feel too bad if you miss out side quests.

As long as you're patient and observant and don't rush the story bosses immediately (which seems to be what a lot of people are doing) you should be able to complete most quests on your own, blind. And yes, taking notes helps a lot.

I'd just follow a guide. there are so many triggers that can accidentally lock you out of a quest that you won't be aware of until you go to continue that quest.
