when you complete the game you can enter "Newgame+" or NG+ which makes enemies and bosses significantly stronger but also increase rune gain, this goes up to newgame+7 at which point it will stop getting harder reaching it's "max difficulty"
other than that it's basically new playthrough but you keep your character and items
when playing multiplayer you can be summoned/invade a newgame+ player, which is most likely what's going on here, as it only takes weapon level and rune level into account iirc
level 165 is a bit past when most people will have finished the game so you can expect to get summoned to NG+ a lot, although really it can happen whenever if the matchmaking lines up, I remember getting summoned for NG+ tree sentinel at level 40
Is there any way to tell beforehand if I'm being summoned into NG or NG+?
Will it get easier over time as I level up more or is NG+ always going to be that difficult compared to NG?
I don't think there's any way to tell other than how strong the enemies are
it will get easier as you level up yeah but it depends on what NG+ cycle you get summoned into, and NG+ is generally always harder than regular NG by a significant amount excluding the first few areas of NG+
