Let's just say you're supposed, for starters, to fight cowardly and aggro as few enemies as possible at a given time.
If you start to equip any shield (100% dmg block is preferred) AND you never totally run out of stamina, you can fight and hopefully survive.
Make note your best move, beside blocking, is to dodge-roll through(!) attacks, since you do have iframes while you roll. Be also wary to non exceed 70% equip load, or you'll have more recovery frames (not a good thing) and overall slower movement.
1.) When not fighting a boss always use 100% phys shields in one hand and use the guard Counter mechanic a lot !
you hold block and just after an enemy strikes your shield you press R2 for heavy attack.
If you do it correctly it does a pling sound.
This makes the game sooo much easier !
2.) There is no explicit goal in this game other than to eventually find and kill all the main bosses.

you can use this to get your vigor up.
early game Vigor should be 2/3 or your level...later half of your level is okay.
So at level 30 you need 20 vigor

This game is not intuitive and beginner friendly on purpose, but it is a good game. You need to try and give it a chance and push through the crap.
I'll give you a starting primer. Go lookup a few dungeons in Limgrave, clear them till you're level 20 something. Then go get bloodhounds fang, the boss is hard, but try rolling to dodge his attacks. After that, go castle morne, fight through the whole thing and kill the boss. After that, upgrade the bloodhounds fang as much as you can. Lastly, go fight margit. He is a genuinely hard boss, even for veterans, so you will. Die a lot. Space yourself out, say an hour or two over several days and you'll learn all his attacks and beat him. If you're able to beat him, you will be able to clear the entire game, because at the end of the day, it's more of the same, learning boss patterns and punish windows.
Don't give up.
It's challenging, punishing, and makes you wonder why you are putting yourself through this when it is supposed to be a "game". But the point is that it's a genre of game whose defining characteristic is specifically to challenge you - and piss you off while doing it.
If you don't like that, well, you're not going to like "souls-like" games. On the other hand, if you die the 30th time, rage quit, then think later "what if I tried this spell, or this weapon, etc" and try and try again - then you'll like it.
If there was one complaint I personally have it's that the games story is convoluted, confusing and there is very little guidance other than a general "go in this direction". The DLC however doesn't even do that. I'm probably in the middle of SotE and have no real idea what's going on lol.
There are also a lot of side quests that require obscure steps to complete and you can ruin the quest if you don't do something at the right time. I really REALLY hate this kind of design - it punishes exploration and discovery (e.g. going to an area or fighting a boss early without summoning a particular helper etc).
As you said, the UI is designed around console and as such is pretty basic. This game - because of the lore, confusing story, names, etc really needed a decent quest tracker.
Because of this, even though you say you don't want to use a guide, I suggest you find a good overview of the game and its side quests. The early game is tough, and you might need to explore certain areas a bit longer than the game suggests to gain some levels before jumping to higher-challenge content (for example, exploring the southern regions).
You've probably discovered by now that there are bosses/minibosses along the so-called progression path that are WAY stronger than you. Don't assume that just because it's "along the way" it's matched to your level, like the Tree Sentinel.
The teleport trap you mention is notorious, everyone falls into it, and panics when it happens because you can't teleport out. It is escapable - you just have to know the path to follow to avoid the enemy and sneak out.
For me the challenge is what kept me coming back to it. There are many many combinations of weapons and skills, and different play styles. You have to experiment, but once you settle on a weapon/style prioritize weapon upgrades focus points into the proper affinities to get the most out of those early levels. Use your spirit ash summons when available, and let it take aggro initially if you can.
