I realised what the problems are pretty quickly.
Try out Opaline Hardtear instead. It is overall damage negation increase. I believe it should stack with bubbletear.
If it's the axe I'm thinking of, I don't think that's suitable for your build. There are much better strength specific focused weapons. The axe from this boss is a quality weapon.
Lionel's set is very heavy and to be honest maybe more suited for a block style of game play over a dodge rolling one. It's good for poise and tanking direct damage, but for Malekith, it's a lot easier with a lighter load out, imo.
More vigor will ALWAYS help because it can save you another couple of hits, but it isn't mandatory. And it isn't the core problem either. People largely blow this out of proportion.
Crimson Amber Medallion is horrendous and I would trade it for something to increase your damage mitigation or stamina regen. I can't see why the haligdrake talisman would be useful for this fight either. If you are going to keep the arsenal charm, then you should go for the greatjar arsenal charm as it's the best version in the game. The reason I don't rate the haligdrake talisman from this fight is because it doesn't do much to mitigate the burning away of HP, this is something that needs to be avoided by running, jumping or rolling. You might also want to grab the strongest possible version of the dragoncrest greatshield talismans if you don't already have it.
Only thing I'd change in your stats is removing 3 points off of endurance to throw into vigor but I don't think that'd matter much either.
If I was going to adopt the usual 'vigor is king' mentality, I'd dump ten or even fifteen points out of strength and throw them directly into vigor.
You can also increase your general damage mitigation by consuming boiled crab or boiled lobster.

@ OP Since you thought being oneshot is the norm for this game. It´s not. You can become quite tanky depending on which build you have. Many people incl myself like using shields from which greatshields offer the best protection but there also other ways to become super tanky.
Today I watched a video from a well known Elden Ring youtuber who pushed a tank build to the max and when I read your post it reminded me of that that and I thought it fits well in here to show you what is possible in Elden Ring (literally everything imagineable)

To be fair, level 116 seem quite low to me for that point in the game.
Of course, people that want to stay at meta levels for PvP is one thing but for those of us that just want to get through the game it should be feasible to be at mevem 150-170 at that point, which means a rather large amount of stat points.
To be honest, that's extremely over the top, lol. 120 is about the norm' for end game.
The only reasons to keep your level low are for the challenge or for PvP.
If you are not into either of those, getting more levels is only beneficial, and you get plenty of runes without farming for them if you explore and do the various dungeons that are all over the place.
It's fine to want to keep yourself at a low level if you have a reason for it, but if like OP you are struggling there is little reason to not go beat some dungeons (and their bosses) for a big chunk of runes and level up.
Another thing to note is the +9 on the flasks, the cap is at +12 and all of the sacred tears are before cumbling farum azula.
It won't magically make you better at the game to have better flasks of course, but healing more with each flask can help when you are struggling against a boss (assuming you have enough time to heal during the fight of course).
Observe his moves and keep trying, try to not only memorize the attacks themselves but also what moves can follow up an attack in a chain, once you start predicting the next move, the tables turn in your favor.
Don't use smaller weapons like daggers to fight him cause you'll miss more than half of your attacks, that was my main issue at the time, my first build was dual daggers.
you need 60 vigor to even think about surviving an attack from most bosses in this game. Especially the DLC.
