Patches?

I cant find patches at the murkwater cave, scenic isle, or volcano manor

1. Are you sure you are at Murkwater Cave? There is also a Murkwater Catacombs nearby.

2. Did you kill Patches during the 'Boss Fight' in Murkwater Cave?

There are no known instances of Patches going missing in the game under normal circumstances.

Note: After Scenic Isle, he is supposed to be at the First Mt. Gelmir grace.

I left him alive and just checked the First Mt. Gelmir grace and found nothing

Interesting that he should just vanish like that. I wonder if there is an undiscovered location that he goes to under certain conditions.

After Volcano Manor, he goes to the Shaded Castle and slumps in the hallway filled with statues right immediately before the Elmer boss battle. He's very easy to miss there.
