You have to visit Caelid first to normally access Altus Plateau. Second half of Dectus Medallion is somewhere in Caelid
If by " start of altus plateau " you mean being teleported to bridge; you can't really progress from that point anyway, you have to use a lift
There's a way to skip it, but that's a different story.
So typically it's recommended to check Caelid first in every meaning.

go to caelid
if you do the quest for the npcs in Caria manor (north west of Liurna) you will be sent to Caelid anyway
Altus is higher level stuff

I'm 100% a casual and could be totally wrong, but I think Caelid is the intended place to go next.

The „skip“ is literally just an alternate path through liurnia and he already accessed altus that way it seems.

To progress story you need to defeat radan anyway. So i always choose Caelid. Plus you know free runes for future.

Yes, Caelid.

Caelid is the first map of the endgame content and Altus Plateau is the second. You can also tell by the difficulty level of the enemies, but in Caelid there are more runes for mobs in comparison. I think that is an intentional opportunity for the player to upgrade.

In addition, there are a number of items scattered around Caelid's caves, dungeons and so on in the field that can be very useful in the following maps. Logical sequence, Caelid first, Altus Plateau after

If you explore everything decently then when you do Liurnia you will end up in Altus after it. Caelid can be made as the fourth map in this way. But you might as well make Volcano Manor as the fourth map and Caelid as the fifth. Liurnia is the kind of map that will let you get to many different maps without getting any medallions. For me it's like this:
Limgrave, Liurnia, Volcano Manor, Altus, Caelid.
-----------
It's easier for me that way. This way an important boss in Caelid will be suitable for your lvl and you will have an epic and fair fight.

Wherever you want, |I went to Caelid first by being teleported there and I did not knew I had a map so I walked my way out back to Limgrave killing stuff and I was some lvl 10 when I was at Caelid.

Don't know why everyone is lying saying Altus is the higher leveled region when Caelid is obviously the endgame area. Yes, the game encourages you to explore Caelid prior to this by giving you half a medallion for an easy access to Altus (you can also bypass this through a quick dungeon), and Ranni's whole quest makes you fight Radahn. But you can very much just skip Caelid entirely and go straight for Altus. The enemies in Caelid are way harder and give you way more runes. But yes, the game does tease you with it earlier on because it's a souls game and wants you to suffer. Personally, I've always gone straight for Caelid at the start of the game and complete everything I can there because I like the challenge.

It's also worth noting that Altus is a point of no return for a lot of quests and skips over a lot of stages for them if you set foot there, but it seems that's already happened for OP. I think it's mostly regarding Ranni, Fia, Rogier and D though.

How do you figure they are lying? How do you think called gives way more runes? Is lyndell in Altus plateau or is it it's own region? I've always considered it inside that Altus plateau region personally but I could be wrong. I don't think that Altus plateau would be a region without including that area though and it contains the early and ultimate late game mandatory content and bosses.

Yeah, Leyndell is mandatory main content that ushers into the end game. But Caelid is still leagues harder than anything Altus and Leyndell has overall. Some enemies and bosses aren't so tough in Caelid, but it has all of the toughest and highest rune yielding bosses prior to Farum Azula. Like, I can go straight for Caelid at the start of the game and completely clear it and end up at level 80 with no rune boosting buffs. Even just killing both Greyolls and the Tree Guardian between them jumps me up to level 50. The Bell Bearing Hunter in Caelid is the toughest in the game and so is the Godskin Apostle. The thing is, there's really not much that's mandatory or even substantial to any questline in Caelid. You start Millicent's quest there, do some stuff for Ranni's quest and that's about it.

Even still, I would still recommend at least a decent amount of exploration through Caelid before Altus. It's just that Caelid is obviously meant to be the end game area that you come back to when your stronger. If you do everything there first, the rest of the game is going to be a trivial.
