Yes. Disable EAC and play offline and you'll be fine.

If you want to go back to vanilla and / or online, COMPLETELY remove the mod(s) and re-enable EAC. If the mod adds custom items to your save file, delete the save file too.

Yes, if you use certain mods and go online with them.

Just disable EAC and stay offline. I haven't really heard of people getting banned as long as their modded character never goes online.

If you plan to play online then typically EAC is going to stop you from playing the game if you are doing anything that can get you banned, The things that can get you banned is unnatural stats or cut content.
