You're not an ally and no gender identity stuff is being forced down your throat. And it is in question. Miquella's journey of stripping pieces of themself to fit an ideal is a queer tragedy story. Miquella and St Trina are the same person. They are one and the same. By claiming they're male you're falling into the narrative's tragedy. It was long established that Miquella would travel as St Trina in the base game and would Identify as a woman during that time. At minimum they're Gender fluid.
Thiollier also mentions St Trina as Miquella's discarded half. Its an interesting dialog where's he talks about wanting to pursue her even as Miquella wanted to abandon that piece of themself.

-Miquella is referred to multiple times as a boy cursed with eternal youth (does not mean he is a kid, as I wrongly expected).
-Malenia talks about him as her brother.
-Miquella's other "avatar" is St. Trina (much as Marika is also Radagon), which is a girl (plant/girl?).
-St Trina tells us how Miquella got rid of her so he could become his mother's replacement as the new God.
-Radahn is the "promised" consort of Miquella. It seems like he was not too fond of this plan, which does not matter since Miquella has the power to steal your heart.
My Conclusion: Miquella is a demigod, so it's hard to call him a boy or a girl. But his avatar certainly resembles a boy while his other avatar (St Trina) would be a girl (girl/plant kinda?). Is he a femboy? Well, physically or gender-wise.. could be. Now, is he really into Radahn? (sexually or physically attracted). not clear. Definitely was the perfect tool as his Lord (as was Marika's Lord, Godfrey), since Radahn is Brutally good with magic and physical prowess.
