I'm not sure if this is because it's from the DLC, or this specific NPC, but it doesn't work for them. Anything DLC-specific that might work?

i accidentally attacked Ansbach in the shadowkeep storeroom because he was just standing there menacingly reading a book (really it was because i was absentmindedly cleaning up all the annoying goblins and shadows and didn't expect a friend there) and he turned hostile.
teleported to church of vows > absolution > friend again.
i think Grandam is unique in her behavior even if you attack her.
she won't remain hostile forever, she will say something about her not being your enemy and you get another chance to put on the divine beast hat and talk to her.
try resting at grace and go back there again.
if you killed her, well, you know the drill.. ng+ in that case because you can't revive npcs.
