Well lots of people seem to be a bit insecure or have some problems with that topic....

Its clear that Miquella is in the body of a boy who looks like a girl , speaks like a girl and charms men to fall in love with him but actually is "adult"
Miquella also has manifested some of his most female aspects in the form of St. Trina.

Just like Marika is able to manifest her male aspects into Radagon.

its also clear that Miquella from very young age choose "Radahn as the guy he wants to have on his side in his life."
Now it is not clearly described what that means.
Consort is usually used when a King and a Queen are married.

Now Consort also means "Partner" which in itself can have 2 meanings.
1.) mate as in buddy/ working colleague
2.) Partner as in men and wife as partners for life.

There are stories of old pagan gods changing their sex at will to ♥♥♥♥ things they shouldn't, I imagine it's a reference to that and not contemporary gender politics.

Miquella carved away the 'St. Trina' aspect of himself, so I imagine he's just a dude at the point at which you fight him.

Edit: Also, as far as I can tell King/Queen Consorts are those who marry into their titles. Marika's children would be called King/Queen Regents, since they'd inherit their titles from her.
