What are some missable items from the base game / DLC, that you can only get through a second playthrough?

I completed a 100% guide, I have every item in the game that you can acquire in a single playthrough, including shadow of the erdtree.

I just started New game+ Now im wondering what am I missing from the questlines that give you 2 options? (Mostly base game but also DLC)

Example: either get Jolan summon or her sword by offering either the Iris of grace or Iris of occultation.

can someone refer me to some type of type of quest checklist of every instance where theres branching rewards, so I can figure out what I am missing and get them in new game+
