Mimic tear never added to inventory

Hi all, since the DLC came out I've gotten the urge to play again.
Never beat the base game so I continued one of my saves which has 80 hrs in it.
Got to sacred grounds and opened the chest with Mimic tear ashes. I didn't immediately use it, but then I got to the ]Valiant Gargoyle fight and no matter how many hours I put into it, there was no consistent way deal with both at the same time. Figured I'd look at some Youtube vids and they pointed out the Mimic Tear Ashe as the best way to fight both at the same time. So I opened up my inventory aaaaand no ]Mimic Ashe. Weird ok, so I go to the storage box, maybe it's there. Nope.

Then I thought maybe I hadn't actually picked it up? Went back to the ]sacred grounds, straight to the chest, and it was already open.

Ok, now I know for sure something is up with the game and not me. I verify game file integrity, delete the game and redownload. Still nothing.

Has anybody had this issue before? Now I'm thinking I might be missing a whole bunch of items.

The save file I was using is over a year old, so maybe some patch fixed this but the save file I used was already corrupted or something?

At this point I'm wondering if I start a new game, will this issue persist? Kinda off putting that such a bug exists after so many years.

You can go into offline mode and just add it with third party program or start over.

I don't want to play offline and modify the save file. More so I'm wondering if this issue is resolved. I'm up for starting over.

No, never and I have multiple chars, all with it (and because I despise Elden Beats and could not care less to fight it aagain).
