If you died without wearing it then your runes as you know would drop, you can equip it after you die to make sure they stay there in case you die again before you pick them up.
Edit: I can drop you some runes if you want.

3. Sacrificial Twig is removed from your inventory
If you have runes lying around from your previous death, they remain there when you die as well.
Doesn't matter what kills you. Note that the Sacrificial twig only works for one death, then it's destroyed.
I mainly used them to do the Super Mario frenzied flame plataforming and while riding towards the orbital strike spamming mausoleum. They work perfectly and you get a message telling you the twig has been used (as Narm mentioned the twig will be consumed and you will have to use a new one). They should work properly everywhere but i dont know if mechanics like deathblight or certain bosses skills bypass the twig effect.
I wonder if i can trade twigs....have a gazillon of them and as max level i dont give a damn about the runes.
