1) Skip around Stormhill Castle in Limgrave to the Liurnia Lake area and get through Caria Manor at the far northwest end to reach Ranni's Rise. At the top you can ask to join Ranni (she'll accept you on the spot if not doing a certain other NPC's quest).
2) Talk to her 3 followers who have appeared as spirits in her Rise, during this time the Rise is made a no combat zone and fog walls surround it to keep you there until you have talked to all 3 and return to Ranni.
3) Start the Radahn festival, either by going up to Altus Plateau (faster, but skips some other NPC quests), or meeting Blaidd who has moved to Siofra River and getting him info to help Ranni by either talking to Seluvis (at his Rise southeast of Ranni's) and taking his letter to Sellen for the info, or beating the boss(es) of Redmane Castle and speaking to Jerren, who gives you info for Iji to then pass to Blaidd.
4) Beat Radahn.
5) Head to the crater formed by the falling star, which gets marked on your map, and it'll take you down to the upper part of Siofra River.
6) In this upper part of Siofra River (after fighting the mimic tear boss) you will need to get through Nokron, making your way down to the giant statue which has a chest with the fingerslayer blade Ranni needs.
7) Return to Ranni, she accepts the blade. Reload the area (quit / load game, fast travel, etc) and the seal on Renna's Rise north of Ranni's will be gone, take the teleporter at the top to the upper part of Ainsel River.
8) Nearby should be a miniature Ranni doll, pick it up, and activate the nearby grace, pick the option to speak to the doll 3 times and Ranni will speak to you and tell you to slay the Baleful Shadow.
9) Proceed through the Ainsel River to Nokstella, at the far end is an elevator down to a passage to the Lake of Rot, but on the way you'll be intercepted by the Baleful Shadow, in the form of Blaidd, defeat it and Ranni will leave you with a key while she goes to do what she needs to do.
10) Beat Rennala in Raya Lucaria Academy to get access to the grand library where there is a locked chest you can use the key on for a Dark Moon Ring.
11) Proceed further past the passage where you fought the Baleful Shadow in Ainsel River to the Lake of Rot, on the other side of which are ruins populated with kindred of rot (those rot shrimp guys), and at the far end of *that* is a coffin sitting at the edge of a precipice, get inside.
12) You'll land by a boss fog door for Astel, beat it and since you have the Dark Moon Ring a seal on the far side of the arena will be gone, giving you access to an elevator up to the Moonlight Altar.
13) Follow the path ahead to the Cathedral of Manus Celes, and look for a hole inside the cathedral to drop down. Inside you'll find Ranni's doll body and some sliced up fingers. Put the ring on Ranni.
14) Ranni's spirit will appear, talk to her, and she'll leave the Dark Moon Greatsword where her doll body was sitting afterwards (not sure if necessary to pick this up, but might as well).
15) Proceed to the end of the base game, and after the last boss, there will be a summon sign by boss's body to summon Ranni, do so and you'll get the Ranni ending.
