Which one, the wings? It's an AoW dropped by a named knight boss found in a fort. To get to it, go to the... Castle Gate grace I think? The one right before the troll knight anyway. From there go south a little bit to the main road, then follow the road south-east, through a dragon, and through a very short tunnel. The fort is right there. Hopefully those directions make sense.

Iirc there are only two transformations... Dragon form and the Lamenter Omen which is in the Lamenters Evergaol.
