Lost every site of grace on first log in the new dlc

So, I just logged back in after over a year of having finish this game. All sites of grace a gone except 3. What's happening?

Yeah... looks like this is the problem. The sad part is that I only played a couple of minutes on NG+

if it helps the rush to get from the first step to dlc isn't that long of a trip, just need to kill 2 guys and do some run around
