Also, to get the Extreme Repentance gesture, I just use my fists. You only have to aggro him, then use the gesture (or die), so the damage doesn't matter.
First, Patches has a long history, appearing in Demon's Souls, Dark Souls 1, Bloodborne, and Dark Souls 3 before making his way to Elden Ring. His persistence in such unforgiving settings and presence across multiple worlds suggests he might be more than a mere human. In the DS3 DLC we get to see a glimpse of his real character, and he's actually a pretty decent guy. Rather, he appears to be a trickster type character who punishes the greedy by luring them into traps.
Despite his deception, Patches is actually one of the most honest characters in a Fromsoft game. He doesn't have any grand plans or ambitions, he doesn't have ulterior motives. In fact, he seems to despise those who do. I think that's part of why he hates clerics in Dark Souls, and why he opposes the Erdtree in Elden Ring. Patches is the one person you can really trust, even if it's just trusting him to kick you into another pit.
