Is there a lore reason why bosses don't respawn when resting at grace? *spoilers?*
Without the rune of death how are we permanently killing demigods? Or am I misunderstanding?

Tarnished are reborn through Grace. Tarnished that have lost their Grace (can no longer see it) die permanently.

Bosses are hewn into Remembrances rather than being reborn. This is specifically done by the Erdtree because the demigods lost their favor and the Erdtree, or perhaps rather Marika herself wants you to become Elden Lord and is doing this to help you become stronger.

Some other enemies like Cruciable Knights are not reborn because they worship the Cruciable instead of adopting the Golden Order's dogma, putting them into a similar situation to the Tarnished.

That's a great blanket lore statement. I'm not quite sure how a lot of enemies fit into it though. For example... say the blood zombies in Mohgwyn's Palace.
