You will need to do a system repair:
open cmd as administrator
run
sfc /scannow
It will show you a response of Windows Resource Protection found corrupt files and sucessfully repaird them, most of the cases is not full repaired so you will need to run
dism /online /cleanup-image /restorehealth
to fuilly repair the system, after that you just restart and run the game, I left the game runing for 30 mins to monitor any issue and it finally stop doing the wierd online only fps.
Now I need to find how to fully disable HDR as dunno why it keeps detecting elden ring a HDR compatible and freaks out my monitor for like 2-3 seconds

In the end, the solution was to create a new Windows profile.
But there are other ways that have helped other players:
1. update NVidia drivers
2. reinstall Easy Anti Cheat
3. uninstall XBox programs
4. deactivate in game overlays (Steam, NVidia, Discord..)
5. check all drivers in the Device Manager and install or update them via the manufacturer's website of the mainboard / graphics card.
6. check event manager for Windows errors
7. deactivate XBox Game Bar
8. check Windows integrity (sfc /scannow, DISM commands...)
9. increase NVidia shader cache size
10. update Windows
11. check game integrity or reinstall
12. set Graphic Priority Setting in Windows Settings
13. Closed EPIC Games in Background
14. closed background processes (wallpaper engine...)
15. disabled raytracing
16. repair WIndows via Inplace Upgrade
17. new Windows profile
I personally upgraded from Windows 10 to Windows 11.
Windows apparently destroys user profiles in the process.
I also had the "GameInput Service" from Windows 10 installed, which apparently had a higher execution priority than the correct GameInput Service from Windows 11. This also led to completely different errors. You can check the event log to see if you are also affected by this. The old service must then be uninstalled and the new one set to start automatically. I believe the Windows 11 service was in System32 and the old one in Program Filesx86 (or vice versa)
Again: Huge shotout to dark-breed. I was complety frustrated and could not find any solution. I already tried half the the things listed above and could not get it to work. IT TOOK MANY HOURS.
I would never even have thought that Windows, let alone the Windows profile, could be the cause. Dark Breed was able to give me a lot of technical insights here and explain these steps to me in detail! He even sent me a self-recorded Youtube video where he shows me how to reinstall EAC and which registry entries I need to delete.
Apparently any game can run properly and you won't notice any problems, but if Elden Ring doesn't work, it's usually still Windows!

This guys crying always this lie "eac is the culprit" should stop to use a PC.
But the Op just stated that disabling EAC fixed the issues :D
Again how can disable EAC fix a online play problem, it can not because without EAC there is no online mode.
He followed my advice and repaired the fault in the windows system.
i ran several fixes and it's running good now with EAC active
