-checking my connected usb hardware individually for the plug-in/plug-out problem
-uninstalling the xbox game app and all associated games
-having ccleaner fix registy errors
-running a full virus scan
-manually reinstalling EAC
-used cmd prompt sfc /scannow

Leaving this up for anyone else looking for possible solutions. Try one of the above ^

I'd like you to try playing without EAC enabled. Many YT videos on how to do this safely

Fortunately I have the seamless coop mod installed and could quickly test how it runs without EAC. Yes, it still happens there too.

EDIT: It happes there too BUT to a much lesser degree, the audio does not cut out during a freeze and it lasts closer to 0.5 seconds per freeze.

Ok just ruling that out

What about disabling XMP, or if it's off, enabling it, in the BIOS? How much RAM do you have? With 16gb I was right at the limits and getting pretty bad frame times(1% lows were in the 20s). After upgrading to 32gb and touching nothing else, it got much better, albeit not completely gone. That much I leave to the developer to address; optimization for this game is sub-standard on PC, and barely passes on console, so much of the performance is in the places often left un-checked.

XMP sets the RAM to the speeds it is configured to run at as per the advertised timings-frequency thereof. For instance, if you have a 3600mhz kit of RAM, and you do not enable XMP, it will default to a known, safe, industry-standardized configuration called JEDEC, where it usually runs at 2133 or 2400mhz - remarkably slower for the sake of absolute system stability. Just turning it on does not present any issues, and in the event your motherboard can't quite run at those speeds, modern mobos can automatically tune the RAM timings to make it work.

If you have no idea what this is about, research is required, particularly because people have been getting scammed over the years buying prebuilt computers that have never been properly set-up with XMP enabled, meaning false advertisements aplenty. So, I suggest some quick YT digging to answer questions about it; from the risks to the whole point.

I did not manage to find the XMP option in the BIOS settings. It's possible i have to run a BIOS update first...
1 thing to check do you have a lot of blocked players? if so that can also cause stutters so i found out

I don't think that's it, I've only played online a handful of times and didn't bother to block any players.

ds3 and dsr stuttered deleted all blocked and its fixed

Not sure why you'd block people in those games since it doesn't stop connections in-game(correction: it works in DSR?)

I had 1 second of lag in every 10 seconds in Dark Souls Remastered.

Clearing Steam Blocked list solved the problem.

the block list goes for all games not just DSR. When you block them its universal block of that person for all games

the block list goes for all games not just DSR. When you block them its universal block of that person for all games
Nah, not for DS1-3. Blocking doesn't prevent connections last I checked
