What causes the phantom/ghost slap?

Was just doing some duels and noticed I was getting slapped silly by ghost slaps(attacks hitting me even though they look like they shouldn't have) way more often than normal. I'm not sure if this is due to me forgetting to turn off my VPN before booting Elden Ring, or if this is something folks learned to control/adapted to in the Soulsborne games.

It's just how Souls games are, if you want actual, consistent, quality connections you'll have to move to Japan, since From doesn't seem to care about proper connections outside of there. You might get lucky and have decent matches here and there but I'd say you'll be saving yourself some trouble not getting too invested in it. You can also limit matchmaking to your region through a menu setting, but it doesn't seem to really work in my experience.
