If that's accurate then that sounds like a stuttering problem rather than a framerate problem.
I tabbed out to do an internet search and the game then re-took focus as full screen with the usual black bars I get with my uw monitor.
It ran fine yesterday, albeit at 50fps, 70% gpu and 50% cpu usage. Today it was 60fps.
That flurry of frames wasn't like it was in 'fast-forward', more 20fps then 60fps. I did read lots of neg reviews about stuttering but for some reason the description 'stutter' didn't come to mind.
If it's the same tomorrow I'll play about with the desktop refresh rate, window/fullscreen mode and my monitor also has a setting that makes it think it's a 27" 16:9 monitor... maybe that'll help. I've just found my first boss... I'll really need it smoother for that!

iframes are tied to the 60 FPS cap.
You can unlock it, but things will get wonky.
I'm playing uncapped with 50% FOV, so the camera is a bit farther, and i don't feel the game breaking.
I got so used having 50% FOV, than playing with the OG close range camera feel weird, easier maybe.
About the OP issues, be sure to also check the vsync and free-sync options in the gpu control panel.
Try various combinations of these settings, alongside locking the monitor refresh rate at 60Hz, check if you get a smoother game-play.
Turn off Vsync (done in the Nvidia Control Panel)
Limit your frames to 60 or 30 (also done in the Nvidia Control Panel)
Open Elden Ring and switch to borderless window
This will definitely improve your experience. Experienced stutters with an RTX3060Ti and Ryzen 7 5800X but after i switched these settings, the game runs perfectly smooth.
