Only bosses and a few minibosses don't respawn. You don't need to kill all this enemies over and over again, just take another way around, run past them or use Torrent and ignore them all.
In case if you die you lose all your runes and need to get to where you died to retrieve your runes, if you die again before that these first runes are 100% lost.

Stealth works at times, but more often than not you'll just be running past them, either on foot or horse, then jumping over a ledge or through a doorway. You can agro a castle full of enemies as long as you dodge roll through their attacks and run past them to safety.
You should kill your way through them initially for a number of reasons, but if you've already cleared and looted an area a couple times there's not much reason to fight them again just to reach the last place you died. Plus you run a high risk of losing your runes that are laying on the ground where you died.
At some point sooner or later you will be good enough at the game to crawl and completely clear an area of the big dungeons and reach the next grace without dying, which is very satisfying.
I wouldn't think so, at least not without major glitches.
You have to rest to burn the Erdtree and i think at least one trip to RTH is needed somewhere along there, too.
