Special on 4 (mouse side button)
Alt weapon is center mouse
Items selection is 5 (mouse side button)
Item use is R
and use e + mouse wheel for pouch
I play with a razer naga. I don't think Elden Ring is playable on a standard mouse.
I realize this is way more than you asked for but I thought maybe it;d be helpful if I explained how I made KBM superior to mouse for me. Lots of tweaking.

Totally doable, but granted.... G Hub software is trash, when all you want is just a simple way to configure buttons and save them directly onto the mouse's onboard memory. They (Logitech) make it so unnecessarily convoluted.
Regardless, it's perfectly playable. After you go in and change all the initial keybindings of course, which I remain absolutely convinced were chosen and playtested by a crab.
