notice how she awaits for his (miquella) return?
would be kinda stupid of her to think he wandered off in his cocoon wouldnt it?
also notice how she mentions how her blood is rotted
her blood ...
now, who is mohg communing with?
its the formless mother -"the outer god of who bestows power upon accursed blood and craves wounds"
... she might know a thing or two about blood, maybe even rotted blood

The rot very well could be eating him from the INSIDE, but like I said if she hadn't pulled that kamikaze move, Radahn would of tanked the bloom and likely came out of it fine.
I don't understand what is hard to understand about this...
Yes, Malenia needed time to recover....but she recovered. Radahn didn't recover his wits, he is progressively getting worse. When you face Malenia she is back on her feet and blooms again. When you fight Radahn, you're putting him out of his misery.
She likely discarded the needle and gave in to the rot god's power in caelid when she realized she couldn't beat radahn in a straight fight, Which is likely how whats his face in radahn's army gets his hands on it.
There is a slight chance that when Melenia went into her coma, Miquella was somehow communicating with her through dreams. Maybe she entered his dreamworld in a hibernation of her own.
It's also possible that when she says waiting for his return that she has no idea where he went and she thinks he may come back. It's possible she thinks miquella is upset at her for giving in to her rot and left out of dissapointment, so she sits and waits hoping for him to return to her.
Also I guess Miquella is a possible time reference as well? In all his statue's he's depicted as being a child, yet in his cuccoon he def doesn't look to be one anymore. I would say that maybe 20-25 years has passed, maybe less. The game world is very inconsistent with the timeframes.

Did Malenia recover enough to regain consciousness and her wits and did Radahn slowly degrade into a husk fighting on instinct or was the narrator at the castle just lying?

he couldnt heal it tho, not yet at least, and its pretty clear he wants to be able to heal the rotting sickness
and when it comes to malenias actions in caelid
considering all i know about her character, hating rot and everything
maybe she actually didnt lose control after all
maybe this was a calculated action
radahn must be stopped to end the age of the golden order and prepare miquellas ascension
so she "took one for the team" so to speak
and again, malenia thinking her brother wandered off in his cocoon seems kinda silly to me

Maybe Miquella realizes that what's happening to his sister is inevitable, that she will be completely consumed by the Outer God of Rot sooner or later.
So he figured the only way to beat an Outer God is with another Outer God. That's why he goes to (or bewitches) Mohg because he knows the latter worships the Formless Mother. Miquella sees this as an opportunity to become the vessel of the Mother instead.
So who knows - maybe we'll get the vessel of the Rot God, Malenia, vs the vessel of the Formless Mother, Miquella, some day
She nukes Caelid and Radahn sort of become a nuke-like himself so what (or wtf do you think a meteor would do to Caelid? And Radahn doing it just dont give a flying f**k about what happens at Caelid as far as he have his combat fun) while Malenia only used it as last resort, not like she was nuking everything everywhere also she avoided using it out of pride too, but yes she sort of killed Radahn motivation while destroying her body on the process but still able to fight, sort of a tie.
Same voice and intonations and bad skin
I'm meaning the narrator stating Radahn's wits are long gone. We know Malenia woke up unless she was sleep walking in the fight against the tarnished.
And Ranni mentioned there not being a victor - but again, her narration is about becoming Elden Lord. Her motivation is not the same as Malenia's. Malenia does not seem to care about the great runes - example of this was that she fought Godrick over an insult and pride, once he groveled at her feet, she left him there in shame with great rune intact. Her motivation for fighting Radahn is unknown - what we do know is the outcome: Malenia lived another day to fight with her wits while Radahn became zombie boi on a pony.
