help with starscourge greatsword

When I equip this a second one appears on my back, I can't set up dual wield.
but some times in the middle of a fight the second sword gets equiped? but only for a short time. Is there a way to keep them both equiped.

You get dual swords when you 2 hand the weapon.

In the game there are some weapons that come in pairs. Most common examples are claws and fists, but there are certain unique weapons that also come in pairs; when you wield them 2 handed, you wield one of them in each hand instead.

I cannot have one in each hand. I can only equip one at a time. but why randomly will they appear in both hands

Not randomly. If you input the command for two-handing a weapon, you'll instead dual wield this one.
