2. nVidia Control Panel -> Program Tab -> Elden Ring -> Power Management Mode -> "Prefer Maximum Performance"

All other settings are at default, so you may want to click the "Restore" button before changing these settings in nVCP.
Download CPU-Z and GPU-Z. Use CPU-Z to verify that your RAM is actually running at 3600MT/s and configured for dual channel. If not, activate DOCP(XMP) in BIOS.
Use GPU-Z to ensure your GPU is operating in PCIe x16. If not, reseat the card. Sag can result in a partial electrical connection, dropping it to x8 or lower.
In Windows Device Manager, try disabling "Microsoft Device Association Root Enumerator".
If you have Gamepass installed, you may have to uninstall all games.
specs are on profile, mine is sort of similar to yours but i have ddr4 ram.
Which areas does it vary the most? Are they grassy areas? Setting Grass Quality to "Medium" can help a lot. When set to "High" or "Max", the game uses physics-enabled grass. Since the game only seems to use one main game thread, the physics grass can overload that one thread.
Another thing to look at is using DDU to completely wipe out all shader caches and old driver entries. Be sure to run it "properly" in Safe Mode to let it do what it needs to do.
Some folks have also mentioned that clearing the temp folders also helps for some reason.

3. Select all and delete. You may get some popups asking for permission to delete. There may also be some files that can't be deleted because they are currently in use. This is normal.
Not sure if Hardware Accelerated Scheduling helps or hurts performance in ER. In my own limited testing, there was no appreciable difference on my nVidia RTX 2060 or AMD W6600 Pro.
I really think the game is just so incredibly poorly optimized that even ultra high end systems will struggle in certain areas. Certain spells and effects have a big impact on performance and it's as if the CPU is doing all of the particle calculations on its own and tracking individual particles rather than using particle groups.

That Walking Mausoleum in the Consecrated Snowfield destroys performance on every system when its "fireworks" begin exploding. When the "fireworks" initially shoot out, the framerate initially drops to 59fps then completely tanks afterwards.
Same goes for vsync. If you can handle a little screen tearing, disable it for slightly higher frame rates and considerably more responsive controls.
The more buffering that is going on, the longer it takes for frames to get displayed. That makes it feel like the controls are lagging. Less buffering leads to faster response time.
Playing on 11600k, 2060(slight OC) 32gb ddr4(300mhz) and samsung 970 evo plus.
Also no amount of nvidia settings tweaking seems to have made any difference.
One question though, should the physx processor set to auto or gpu and would that make a difference?
Plenty of placebo fixes suggest manually setting the PhysX and preferred CUDA GPU. It makes no difference as ER utilizes neither.
Its not a critical issue, its a performance and optimization issue, which, in the post-launch period we're in, is when it would normally be addressed. Also, just Google the stuttering and frame drop issue and you get plenty of threads. Buddy we're also in a thread about people having those issues. Knock off the hyperbole; did you show up just to be obnoxious?
